What you'll be doing
While working within the National Framework for NHS Continuing Healthcare (CHC) and/or the Care Act 2014, you will chair Multi-Disciplinary Meetings (MDTs) and conduct reviews and assessments on behalf of Integrated Care Boards (ICBs) and Health Boards. These assessments will be carried out with the individual, their provider, and family (or their representatives), all conducted virtually via MS Teams or Zoom.

- Able to communicate very sensitive, complex condition-related information to individuals and/or relatives where there may be barriers to understanding, requiring persuasive and influencing skills, and demonstrating empathy and reassurance.
- Highly skilled in engaging others in collaborative working through effective negotiation, persuasion, and interpersonal skills.
- Being able to deal with highly sensitive, highly complex information in relation to changes to service provision.
- Excellent written communication skills with the ability to produce reports that can be understood by both individuals, their representatives and clinical audiences, often in relation to complex or contentious subject matter.
- Analytical and judgement skills for assessing and interpreting specialist conditions.
- Excellent planning and organisational skills.
- Self-motivated and experience of achieving KPI’s.
- Able to liaise with other services and professionals, both internal and external.
- Experience of planning provision of care for individuals.
Experience and qualifications required
Essential:
- NMC Registered Nurse.
- Minimum 12 months post qualification experience.
- Previous experience working within an NHS Continuing Health Team or in the community completing CHC assessments and reviews.
- Experience of chairing MDT meetings.
- Ability to write clear and comprehensive reviews and assessments to a high standard.
- Able to complete person-centred care and support plans.
- Computer literate, specifically for Microsoft suite including Outlook & Excel.
- Experience in similar work; reviewing, evaluating, and planning adult health and social care.
- Good working knowledge of CHC Framework 2022.
- Knowledge of Care Act 2014 and Mental Capacity Act 2005.

To support this mission, we’re expanding our team and are thrilled to offer a fantastic opportunity for a Registered Nurse to train as a Continuing Healthcare (CHC) Practitioner.

About the Role
As a Continuing Healthcare (CHC) Practitioner within our Liaison Care Delivery Team, you will receive full support to develop the knowledge and skills necessary to excel in all aspects of the Continuing Healthcare process, in accordance with the NHS CHC Framework. This includes completing assessments using tools such as the Decision Support Tool, Fast Track, and Adult Needs Assessments.
Key Responsibilities upon satisfactory completion of training include:
- Lead Multi-Disciplinary Team (MDT) meetings.
- Completing either the NHS Continuing Health Care (CHC) Decision Support Tool or Adult Needs Assessment with clear eligibility recommendations.
- Completing CHC reviews.
- Manage relevant follow up calls and request further documentation and evidence as required from other professionals and care providers.
- Conduct the assessments, reviews, and any care and support plan, working to both Liaison Care and the ICB’s Key Performance Indicators (KPI’s).
As a Continuing Healthcare (CHC) Practitioner, you will have access to:

Who We're Looking For
- Registered Nurse
- Minimum of 12 months post-qualification experience
- Experience working in adult health settings, including completing person-centred assessments and reviews
- Strong writing skills, with the ability to produce clear, comprehensive reviews and assessments to a high standard
- Ability to complete person-centred care and support plans
- Computer literate, with proficiency in Microsoft Office Suite (Outlook & Excel)
- Experience in reviewing, evaluating, and planning adult health care
- Diploma or degree in nursing or equivalent (but must be NMC registered as 1st Level Nurse)
