Job Summary

We are seeking an experienced Learning Disabilities Assessment Manager to join our Liaison Care Transformation team, supporting the delivery of complex Continuing Healthcare (CHC) assessments for our client.

In this role, you will undertake complex Continuing Healthcare (CHC) assessments for individuals in receipt of CHC funding. You will review clinical records, assess ongoing support needs, and provide evidence-based recommendations to ensure appropriate levels of care and funding. Working closely with commissioners, providers and Local Authority representatives, you will support the redesign and implementation of person-centred care packages, develop clear KPIs for future monitoring, and produce robust clinical reports to support decision-making, appeals and complaints. You will also ensure care plans are updated with a focus on meeting assessed needs and preventing avoidable hospital admissions.

Principal Tasks and Activities: 

  • Conduct Assessments – ensure relevant paperwork is collected and shared with appropriate representatives prior to assessment, undertaking clinical assessments with individuals, representatives Providers (as required) and relevant Local Authority representatives.
  • Evaluate individuals needs based on medication records, care plans, daily living activities, discussions with Providers.
  • Provide Clinical Advice – provide clinical expertise to the process of assessment, care planning and monitoring of care.
  • Reviewing Cases to ensure they continue to be eligible and are structured according to need with appropriate levels of designated resource.
  • Determine eligibility between Health and Social Care and make recommendations (taking account of the views of the Local Authority representatives) on eligibility and appropriate level of support related to clinical and social care needs.
  • Complete Assessment Documentation and ensure Local Authority comments are incorporated.
  • Ensure Care Plans are updated based on Assessment recommendations.
  • Draft Recommendations for ICB to consider clear supporting evidence.
  • Supporting Patients – Provide information and support to individuals and families through the assessment process.
  • Provide evidence for all cases where there is a complaint.
  • Collaborating with other professionals.

Job Summary

We are seeking an experienced Mental Health Manager to join our Liaison Care Transformation team, where you lead complex clinical assessments, case reviews and package redesign activity across a range of health and care workstreams. Using specialist knowledge of Continuing Healthcare (CHC) and Mental Health services, you will undertake assessments, provide evidence-based recommendations, support commissioning decisions and help ensure individuals receive safe, effective and person-centred care.

This role requires a confident and autonomous clinician with strong professional judgement, excellent case management skills and the ability to deliver high-quality outcomes within a fast-paced and evolving environment.

Principal Tasks and Activities: 

  • Undertake complex Continuing Healthcare (CHC) assessments, reviews and case evaluations, applying the National Framework and relevant guidance to support robust clinical decision-making.
  • Lead the review, analysis and redesign of complex packages of care, ensuring recommendations are clinically appropriate, outcome-focused and represent value for money.
  • Complete comprehensive clinical assessments and produce high-quality reports with clear rationale, evidence-based recommendations and defensible professional judgement.
  • Apply advanced clinical reasoning to evaluate individuals’ health needs, levels of risk and support requirements, making recommendations regarding care planning and funding arrangements.
  • Manage a complex caseload autonomously, prioritising workload effectively and delivering outcomes within agreed timescales and quality standards.
  • Provide specialist clinical advice and support to commissioners, providers and partner organisations on complex Continuing Healthcare and Mental Health cases.
  • Assess and review individuals with mental health needs, ensuring care arrangements are appropriate, effective and promote improved health and wellbeing outcomes.
  • Support the review and management of Section 117 and other Mental Health-related cases where required, providing clinical expertise and recommendations.
  • Work collaboratively with Integrated Care Boards (ICBs), Local Authorities, providers, service users, families and multidisciplinary teams to support effective assessment and care planning processes.
  • Develop and monitor measurable outcomes and recommendations to support ongoing review and quality assurance of commissioned services.
  • Contribute to service improvement, transformation and quality initiatives across multiple workstreams, demonstrating flexibility and adaptability to changing organisational priorities.
  • Ensure all assessments, reports and documentation are completed accurately and professionally, in accordance with regulatory, professional and information governance requirements.
  • Provide evidence and professional justification to support challenges, appeals, complaints and complex decision-making processes where required.
  • Build and maintain positive working relationships with key stakeholders, demonstrating credibility, professionalism and clinical leadership in all interactions.

Experience

Essential:

  • Registered Mental Health Nurse (RMN) with significant Continuing Healthcare (CHC) assessment and review experience.
  • Proven ability to manage complex cases, make sound clinical judgements and produce high quality evidence-based recommendations.
  • Experience working autonomously across multiple workstreams, delivering outcomes and managing key stakeholder relationships.

Desirable:

  • Experience of Section 117 Aftercare assessments and reviews.
  • Knowledge of the Mental Health Act, Mental Capacity Act and Care Act.
  • Experience in commissioning, service transformation or complex case review programmes.

Skills & Competencies

  • Strong clinical assessment, decision-making and report-writing skills, with the ability to make robust evidence-based recommendations.
  • Excellent knowledge of Continuing Healthcare (CHC) processes and the ability to manage complex cases autonomously.

  • Experience working across multiple programmes, workstreams or transformation projects.

Key Responsibilities

While working within the National Framework for NHS Continuing Healthcare (CHC) and the Care Act 2014, you will chair Multi-Disciplinary Meetings (MDTs) and conduct reviews and assessments on behalf of Integrated Care Boards (ICBs) and Health Boards. These assessments will be carried out with the individual, their provider, and family (or their representatives), all conducted virtually via MS Teams or Zoom.

Experience

  • Previous experience working within an NHS Continuing Healthcare team is essential.
  • Ability to write clear and comprehensive reviews and assessments to a high standard.
  • Able to complete person-centred reviews, assessments, and care and support plans using evidence-based information.
  • Robust working knowledge of CHC Framework 2022.
  • Working knowledge of the Care Act 2014 and the Mental Capacity Act 2005 and how these relate and interact with the CHC Framework 2022.
  • Computer literate, specifically for Microsoft suite including Outlook & Excel.

Skills & Competencies

  • Able to communicate extremely sensitive, complex condition-related information to individuals and/or relatives where there may be barriers to understanding, while demonstrating empathy and reassurance.
  • Highly skilled in engaging others in collaborative working through effective negotiation, persuasion, and interpersonal skills.
  • Being able to deal with extremely sensitive, highly complex information in relation to changes to service provision.
  • Excellent written communication skills with the ability to produce reports that can be understood by both individuals, their representatives, and clinical audiences, often in relation to complex or contentious subject matter.
  • Analytical and judgement skills for assessing and interpreting specialist conditions.
  • Excellent planning and organisational skills.
  • Self-motivated and experience of achieving KPI’s.
  • Able to liaise with other services and professionals, both internal and external.
  • Experience of planning provision of care for individuals.

Qualifications

  • Registered Nurse with at least 12 months post-registration experience.
